What to Bring
To ensure your first visit goes smoothly, please bring the following items:
- Referral slip and any previous X-rays (if applicable)
- Complete list of current medications
- Dental insurance forms and information
- Medical insurance forms (if applicable)
- Valid photo identification
- Method of payment for any co-pays or fees

X-Ray Requirements
To provide the most comprehensive care:
- Recent X-rays (taken within 6 months) can be transferred from your previous dentist
- Additional X-rays may be taken at our office if needed for a complete assessment
- Digital X-rays provide detailed images with minimal radiation exposure
